Eligibility
To be eligible for our services, an individual must be a resident of Indiana and be receiving or about to receive treatment for breast cancer as defined below:
- Radiation Therapy*
- Chemotherapy
- Mastectomy (Post-Operative)
- Extensive Surgical Procedure related to Breast Cancer
- Biologic Therapy for Metastatic Breast Cancer
Exclusions for eligibility: Oral therapy (e.g. Tamoxifen) or lumpectomy as single treatment.
